They can't assume that.I have 2 business cards already then been paid off long time ago and no missing payments.There is really no reason to deny a card unless they don't wanna give person to use their credits for 0%.As you said paid cards not really a paid cards tho, bonus covers the paid fee and then card next year can be downgraded to non paid card.Thats could be just luck at this point.

I've never received one on any of my ink cards! Like pcman2000 said its likely you received a referral bonus.

For me, my limit is only $4,000. I asked for increase and got denied due to account age length.
I need the credit increase because my plan was to pay for my entire HVAC system replacement on the card and take advantage of the 0%. Now I'll need to charge it to the $4,000, pay off roughly half of it, and then max it out so I can hit the $6,000 bonus.
