US Alliance offered that for a few years, but more recently that dropped to 2% everywhere cash back.
I'm not aware of any card offering 3% cash back on everything anymore.
You can get a bit above 2% cash, but only if you park substantial cash/investments in Bank of America accounts. More info about rewards cards here:

Yes, you can do this. Yes, most people usually pay their card only once per month. If you pay your statement balance every month, you pay no interest. Outside of special circumstances (like trying to earn a bonus with a $500 credit limit, or to get a temporary credit score boost), there's rarely a reason to pay more than your statement balance once per month.
I'm as much a fan of Citi as I am of any major bank, in that I find all of them to be scummy as fark, but there has never been an issue with any CC rewards, points, miles or cash...
Wells Fargo cash card offer is still active. I've had it for over a year now (off the deal posted here a while back). It's $200 cashback with ONLY $500 of spending limit.
Also, its 2% instant cashbank for EVERY purchase. I use it for bigger purchases, then pay it off before the 1st bill and then enjoy the cashback to buy other stuff.

I used a virtual number on a slightly sketchy site (I set a very low limit and 1mo expiration) and ended ip with a fraudulent charge. No big deal right? I reported it to Citi fraud dept and they canceled the original card (despite my objections, it was their "policy") including all virtual numbers tied to it! I was left without a card for over a week until the new card arrived. It really was inconvenient because I had a lot of autopay bills on that card. IMO it defeats the whole point of virtual cards.
Does it include the virtual card numbers Citi feature?
Source : I've had the card for years and don't use it much since they removed those features.
I've been eyeballing a 3% back on all PayPal purchases via the PayPal credit card. Not quite everything, but it could be pretty significant if your arranged your purchases appropriately.
with no categories to track or quarterly activations". It does specifically exclude things like money orders, gift cards, and casino charges, but I've gotten 2% on everything, including gift cards.
Just taking a random guess, but maybe you ended up with the Autograph or Attune which has categories and 1% on everything else?
